Abstract

Introduction: moderate preterm (MP) and late preterm (LP) infants are more likely to require delivery room (DR) interventions compared to term infants. However, in Argentina, there is a lack of local data regarding resuscitation requirements in these populations, as no previous national studies have described their delivery room management.
Objectives: to describe the frequency and type of delivery room interventions in MP and LP infants, and to analyze the association between the level of intervention and neonatal clinical outcomes.
Methodology: aprospective multicenter cohort study was conducted across 42 maternity hospitals in Argentina from August 3, 2021, to November 27, 2022. MP (29 0/7–33 6/7 weeks) and LP (34 0/7–36 6/7 weeks) infants without major malformations were included. Newborns with major congenital malformations or a prenatal decision for limitation of life-sustaining care were excluded. Six delivery room intervention levels were classified. Maternal and perinatal factors, as well as clinical outcomes at discharge, were analyzed.
Results: a total of 1,747 infants were included: 690 MP and 1,057 LP infants. Overall, 72% of LP infants required no delivery room intervention, and only 0.09% received advanced resuscitation (AR). In contrast, 72% of MP infants received some form of intervention, and 2.17% required advanced resuscitation. Higher intervention intensity was significantly associated with lower gestational age, lower birth weight, incomplete prenatal care, and increased neonatal morbidity and mortality.
Conclusions: moderate preterm infants present a higher need for delivery room interventions. Advanced resuscitation procedures are associated with worse clinical outcomes.
